• Text us your name, company name, zip code of your patient. We will reply promptly whether we can cover your patient
• Fax to us at 213.250.5578 your patients demographics, which includes patient name, address and phone number.
• If you don’t have a contract with us yet, kindly fax to us your company fax cover sheet. Make sure your company name, address and tel# and fax# is on your fax cover sheet. We will make a contract and fax it to you along with personal guarantee form for your review and to be signed and to be faxed back to us. Once contract & personal guarantee form is completely signed and all requirements given, then we can start seeing your patients.
